Below are the services provided by Dr. Putman or his office. Fees listed are the cash fees. We do accept United Healthcare insurance and Value Options (a subcontractor mentioned on the back of your insurance card) and rates and co-pays for insurance supported visits are determined by the carrier.
We see many patients who do have other types of health insurance. Check with your insurer to determine what your out-of-network coverage is and how much they will reimburse you for your cash payments to our office. We will provide receipts for you to give your insurer to receive any reimbursement they provide.

Psychiatric Evaluation
Dr. Putman begins with a thorough assessment of symptoms, history, medical condition and lifestyle and then establishes a diagnosis.
You and he then proceed to methodically and carefully evaluate various diagnostic and treatment options to bring the identified symptoms are under control.
A long term plan is developed to prevent recurrence of the symptoms as much as possible.
Recommendations and treatment options are based upon the prevailing responsible scientific literature.
An initial evaluation lasts about one hour. This type of evaluation is also appropriate for second opinions and Independent Medical Evaluations (IME).
The fee for Psychiatric Evaluation or Second Opinion is $250. Please contact the office for IME fees.

Medication Management Visits
Patients who have already been evaluated by Dr. Putman and who are in a mutually agreed upon treatment plan with him would schedule this type of visit to monitor and discuss treatment progress involving medication and only limited psychotherapy.
Often, patients scheduling these visits may be in psychotherapy with other, non-physician therapists.
Fees are $95 for 15min and $130 for 30 min.

Individual Psychotherapy
When indicated, Dr. Putman may prescribe a form of this psychotherapy. Sessions will last one hour and will usually occur weekly or every two weeks. Fee is $180.
Rarely, a half hour session may be appropriate and the fee is $130.
